Our Youth ministry group is for 8th grade through seniors in High School that gather monthly for social and service events. Please See the Youth Ministry Calendar for monthly updates.

No cost to join this group. However, some events do have a fee.

Medical Authorization: By checking this box I grant permission for the administration of First Aid to the participant by the people in charge of this trip and those transporting the participant to and from as their judgement deems advisable, and to make the necessary referrals to qualified physicians for treatment of illness or accidents of a more serious nature. I understand that I will be promptly notified in the event of any serious illness or accident and prior to any major surgery, except when delay in such communication would endanger life. In case of medical emergency, I understand that every effort will be made to contact the emergency contact of the participant. In the event that I cannot be reached, I hereby give permission to the physicians selected by the adult staff/volunteer to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery, if deemed necessary for the child.
Authorization: By checking this box I give permission (for the participant) to attend events I hereby release and indemnify, the Diocese of Joliet and Maternity BVM Catholic Church in Bourbonnais, it's staff and volunteers from any and all liability arising from claims of any kind of nature whatsoever from participating in this event. I understand that the above participant is responsible for following certain rules and regulations that are designed for their safety while taking part in this event. Authorization By checking this box, I understand that any work I provide is strictly voluntary, and I (or my business) will not be compensated or credited in any way. I grant permission for the participant to be photographed or videotaped, which may be used for future promotional efforts, including the MBVM website, social media and/or newsletter bulletin. As well as permission to participate in virtual platforms for the purpose of programmic content.
Hold Harmless: By checking this box, I agree on behalf of myself, my heirs, assigns, executors, and personal representatives, to hold harmless and defend Maternity BVM, the Diocese of Joliet, its officers, directors, agents, employees, or representatives from any and all liability for illness or death arising from on in connection with my participation in MBVM events. In the event that I should require medical treatment and I am not able to communicate my desires to attend physicians or other medical personnel, I give permission for the necessary treatment to be administered.
CODE OF BEHAVIOR: I acknowledge that I am representing our diocese/parish during this event, and I will represent us well. I will adhere to all Diocesan Guidelines and display responsible, mature, and respectful behavior in my words, actions, and usages. EXPECTATIONS 1. All participants are expected to arrive on time. 2. All participants are expected to demonstrate respect and common courtesy at all times. Inappropriate language/behavior/conduct will not be tolerated. No use of cell phones unless you ask the adult in charge. 3. Socializing should always be done in public areas. 4.Dress should reflect the values of modesty and respect, and inscriptions and images on clothing should reflect Christian values. 5. The possession or consumption of any alcoholic beverages is prohibited. 6. The possession of any illegal substances is prohibited and subject to legal action. 7. Smoking, vaping, e-cigarettes, smokeless tobacco, and cannabis in any form are prohibited. 8. Weapons and/or drug paraphernalia are prohibited. 9. Stealing or damage to property is NOT tolerated. INFRACTION OF THESE RULES CAN MEAN IMMEDIATE DISMISSAL on a case-by-case situation. Youth will be written up and disciplinary actions will take place in the example of: writing a letter, paying for damages or theft, being dismissed from the youth ministry.
